Biography

David Perlmutter was born on December 31, 1954, in Coral Gables, Florida, the son of Miami neurosurgeon Irwin Perlmutter. He earned his MD from the University of Miami School of Medicine and became a board-certified neurologist with a legitimate clinical practice in Naples, Florida. He also holds fellowship and certification from integrative and holistic medicine organizations, credentials that signal a departure from evidence-based standards.

Perlmutter published his first major popular work, Grain Brain, in September 2013. The book argued that gluten and dietary carbohydrates are the primary driver of Alzheimer's disease, Parkinson's, depression, ADHD, autism, and a sweeping range of other neurological conditions. The book became a New York Times bestseller and was ultimately published in 32 languages. Its commercial success established Perlmutter as one of the most prominent celebrity doctors in the anti-grain movement.

The scientific reception was sharply negative. Yale epidemiologist David Katz described it as a 'silly book' representing conclusions 'way ahead of any justifiable evidence.' Alessio Fasano, the world's foremost celiac disease researcher, said the book was 'full of exaggeration and generalization.' Microbiome researcher Jonathan Eisen, commenting on Perlmutter's follow-up book Brain Maker, stated that recommending gluten-free diets and probiotics as cures for serious neurological disease was 'idiotic.' Reviewers found that Perlmutter relied on selective citation of weak studies, ignored the large body of evidence linking whole grain consumption to reduced dementia risk, and overgeneralized celiac disease findings to the general population.

In 2023, it was discovered that Grain Brain contained passages plagiarized from The End of Illness (2012) by David Agus and Kristin Loberg — Loberg having been the co-author of Grain Brain as well, raising questions about the writing process. Perlmutter has also recommended that parents request spread-out vaccination schedules for children, a position that contradicts CDC, AAP, and WHO guidance.

Perlmutter sells supplements through his website, serves as an Associate Professor at the University of Miami Miller School of Medicine, and continues to publish books and maintain a large social media following. He represents a concerning archetype: a genuinely credentialed specialist (board-certified neurologist) who has leveraged that credential to promote dietary claims that far exceed what his specialty training supports or what the evidence base justifies.

Claims & Debunking

Gluten and carbohydrates are the primary cause of Alzheimer's disease, Parkinson's, autism, ADHD, depression, and most neurological conditions.
DEBUNKED

Perlmutter bases this on selective citation of weak studies. The causal claim contradicts decades of epidemiological evidence. Whole grain consumption is consistently associated in large prospective studies with reduced dementia risk. Aleissio Fasano, the world's leading celiac researcher, called Perlmutter's book 'full of exaggeration and generalization.'

Most people have unrecognized gluten sensitivity that is silently damaging their brains.
MISLEADING

Scientific research shows that only up to 13% of the population may have non-coeliac gluten sensitivity, and the evidence for neurological damage in this group (outside of confirmed celiac disease) is not established. Perlmutter treats this as near-universal and uses it to recommend population-wide grain elimination.

Parents should ask pediatricians to space out children's vaccinations.
UNPROVEN

Perlmutter has promoted vaccine schedule hesitancy, which contradicts the recommendations of the CDC, AAP, and WHO. Spacing out vaccines leaves children unnecessarily exposed to preventable diseases during the gaps and is not supported by evidence of reduced risk.
